The history of the child's development - form 112 / U

One of the earliest documents that are issued for a newborn child is the registration form No. 112, in which the entire history of its development is recorded. This card is filled by a pediatrician on the first visit to a young family who returned from the hospital, or when the mother first contacts the baby with a baby in a pediatric outpatient clinic.

Today, in most medical institutions in Russia and Ukraine, the developmental history of the child in form 112 / U is immediately recorded in a special card. This "book" is issued to every family completely free of charge, since for its production and provision of new parents to it, the state bodies are responsible.

What data is entered in the registration form number 112?

Form No. 112 / U is approved by the government, therefore deviations from it are not allowed. In each medical-prophylactic establishment at the initial registration of the baby's card and taking it into account, the following information is entered in this registration form:

In the future, at each visit of the child by a pediatrician or at the hospital in the form 112 / U, the state of health of the crumbs, complaints of young parents and medical measures taken in connection with the disease are described. In addition, it is mandatory for this form to include information about the vaccination and why the vaccinations were not done.

In the process of dynamic observation of the child, the doctor also enters information about the neuropsychic state, the dynamic development of the baby from the physical and emotional side, recommendations on diet and nutrition, as well as the results of the examination of specialized specialists.

The medical card must be stored in the children's polyclinic until the patient is 18 years of age, but if necessary, parents can at any time receive a copy of the child's development history in form 112 / U. When transferring a young man or a girl to a preventive treatment facility for adults, this card remains in the vault of a children's polyclinic for a period of at least 25 years.

Meanwhile, a pediatrician must make a stage epicrisis on the basis of the information entered into it, which is sent to the adult polyclinic.
