At the second stage with these documents, you need to apply to the Ministry of Health of your region for an IVF fertilization free trial. The following documents must be submitted to the Ministry of Health of the region:
- the application is completed on the standard form, where the full name, residential address, series and passport number and phone number are indicated;
- a copy of the passport of a Russian citizen;
- copy of discharge from the medical institution;
- the patient's consent to the use and processing of his individual data.
After receiving a written application for a free IVF, the answer must be ready no later than 10 days later. The decision is made by a special commission. If the commission does not provide a quota to a patient who has all the indications, then such a patient is put in the queue for free IVF before receiving quotas. An extract based on the results of the commission's decision is issued to the patient in his hands or sent by post.
If the commission has approved a positive decision to conduct a free IVF, then the third stage of obtaining a quota begins here. The representative of the commission must give the patient a voucher for artificial insemination and send the documents to the medical center where the IVF is done free of charge.
